locally from an optical store (privately owned) with a wonderful reputation. My prescription is rather complicated, and my opthomologist has stated strongly not to go to any of the mall stores. He does not have a store of his own, and recommends about 3 or 4 in the area in which he has a lot of confidence. He has told so many horror stories about the outlets I am kind of afraid to even try them.

From some guy in a dark alley. As long as you don't ask where he got them, you can get some really good lenses for cheap, but beware, his El Camino smells kinda funny and I don't think he's a licensed optometrist.

Local opthomologist. I don't completely trust big chains. I have never had a bad experience with one, I have just always gone to local places.

I was having this same conversation at work this week.
The consensus among us was that we prefer local optometrists and only go to a chain if we know of a local branch that is decent.
For the actual glasses, we like to use a store/optometrist with a good selection to get our primary set of glasses, then use discount websites to order several pairs of cheap backups.

I have personally used America's Best recently because I was broke and I have a fairly easy prescription, but I still want to find a good local optometrist.

I second @kalicat83's comment. For my primary pair of glasses (and for contacts, which I wear every day), I use my local eye doctor that I've been going to since I was little. But I've used EyeBuyDirect for a back-up pair of glasses, which I was very happy with in that capacity--although I will advise that for people who go that route, see if your regular eye doctor will give you your pupillary distance. I could never accurately measure it myself, and if that's off on your glasses prescription, you're going to give yourself a massive headache.

I feel like for a primary pair of glasses, the expense of going through a local, trusted eye doctor's office is worth it to ensure proper fit and prescription.

I went to the optometrist at costco once. It wasn't as gross as the scene in "minority report" where tom cruise gets his eyes replaced and accidentally eats a moldy sandwich, but it was similar.
