I have never understood this about the name of this restaurant: How does one "grill" macaroni? Why call it that?

Because the word doesn't mean curvy noodle. Here's the history of the term:

Elbow Macaroni gets its name somewhere else entirely. Of course the feather in a cap being macaroni was mocking by the British that we've adopted as our own song. That's far from actually being fancy enough to truly call "Macaroni"
