You need an account at Fab.com of course to claim the credits. It's a pretty legit site, I've bought random things from them since the beginning.
An added bonus, if you have an account with them you can do this survey from an e-mail I got for $5 credit (totaling up to $20 with all the credits here, if my math is correct). Thank you! I found out about Fab from the Wooters, have ordered twice and found not only great products, but really unique ones. Packaging was classy, too, which automatically makes me look so.

I've bought items from Fab numerous times, the shipping is slower but getting better. Great items and very carefully packaged. Probably my favorite flash sale site on the interwebs.

just submitted both and went to check out and no discount... nothing showing under credits or anywhere. do they show up after hitting submit order?
Still no credit has shown up for me. Clicked the links over an hour ago and said they were added but nothing!
Still waiting on the survey credit, I guess it said 24 hours, but I am curious whether they at least send a confirmation email after you complete it and give them your email address. I've received nothing in the way of confirmation for completing the survey, but hopefully the credit is there tomorrow.

Should take around 30 minutes or so for the first two links (having said that you also have an account when you click them; doesn't hurt to re-click the links) and will show up on https://fab.com/my-credits/.
Same here. Just checked today after I woke up, still nothing.
Bit I figured this would take longer since its a third-party application. My best guess is that they have to this all manually.
I just went to Account > My Credit > Shows $15. Still waiting for the survey credit but it said it could take a day.

I just checked my credits and the $5 survey credits were posted! $20 in total now! :]
So that took around 12 hours (or less).
The survey credits expire on Dec 31 2013.
Looks like the survey is dead but I am waiting on the $10 and $5 credit to show up.

Did both links, got both pop-ups saying I have credit, but 'My Credit' link shows no credit :(

It's not instant. Give it around 30 minutes for the first two links for the credits to show up.
$15 showed up on my account. Not sure if it was because I never had an account before today, but it took several hours for the credit to appear for me. We'll see tomorrow about the survey.

"This survey is currently closed. Please contact the author of this survey for further assistance."

Yeah, I saw that link also. I tested it a few hours back but still haven't gotten the credit from that link.
It says that it applies directly to the cart, but nothing.
It's either I've gotten too much credits or haven't waited long enough.

I did the survey 24 when I saw this yesterday morning. Did not get anything.
I also clicked on the 5 and 10 nothing.
This morning I clicked on the 10 and got 10 (see your cart)
But the 5 tells me I am not able to use it.
I signed up yesterday as a new customer.
I'm not so sure about what to say about that... :/
By the way @hikuro, the extra $5 link were just posted. I'm at a total of $25 in credits! Expires Jul 24 2013.
looks like they fixed the loophole - if you sign up for one offer, you can't use any of the other ones. Luckily I used the $10 one first!
Yes, it appears you can only use one offer now. After using the $10 offer, I got this message for the $5 offer: "Sorry! Your account is not eligible for this Special Offer."
